• Prescription and/or over-the-counter medication WILL NOT be given at school without the "School Medication Prescriber/Parent Authorization" form completed by the physician and the parent. The form is printable from this site or available at school. 
    • All medications should be brought to the school by the parent or a designated adult. Students must not bring medication to school. 
    • Prescription medication must be in a pharmacy container with a properly authorized pharmacy label. 
    • Over-the-counter medication must be in a new, unopened bottle/container. Over-the-counter medication must also have a medication prescriber/parent authorization form completed by parent and physician.
